You pray about something, life keeps moving, and a few weeks later you can't remember exactly what you asked for. Or how the Lord showed up. This free prayer journal printable fixes that with one simple page.
It's a single-page prayer request and praise report. You write down what you're bringing to God, and later you come back and write down how He answered. That's it. No fancy system, no pressure to do it a certain way.
I made this printable because of how I used to keep track of prayer requests. I usually save them in the Notes app on my iPhone so I remember who to pray for. I don't like telling someone "I'll pray for you" and then counting on my memory to follow through. So I either pray right then and there when I hear about the need, or I write it down to pray over later.
The problem is that my prayer requests ended up buried between grocery lists and random ideas. I wanted a set-apart, structured place for prayer requests and praise reports, so I created this prayer request printable. Now everything lives in one spot, and I can look back and see how God answered.

What's on this prayer request printable page
I kept the layout simple so you can fill it out in a few minutes. Here's what each section is for:
- Date: Write the day you started praying about it. Future you will be glad you did.
- What I'm Bringing Before God: A big open box for the request itself. Write it out in plain words, the way you'd say it to Him.
- What I Sense God Is Saying: A spot for anything the Lord puts on your heart while you pray. It's fine to leave it blank at first.
- Verse Anchor: Pick a verse to stand on while you wait, and write it here.
- Praise Report / How He Answered: Lined space at the bottom for when the answer comes, so you can look back and see it in your own handwriting.
It's one page, US Letter size (8.5 x 11), as a PDF. The design is soft lavender watercolor flowers and leaves, calm and clean, nothing busy.

How to use your prayer journal printable so it sticks
A page only helps if you actually come back to it. A few things that make that easier:
- Print a handful at once. Keep a small stack where you pray so you're not hunting for the next page.
- One request per page. It keeps things clear, and the praise report at the bottom stays tied to that one prayer.
- Choose your verse anchor first. If you're not sure where to look, Bible Gateway lets you search by word or topic. Write the verse out by hand. It sticks better that way.
- Keep "what I sense God is saying" short. A line or two is plenty. Then hold it up against Scripture and keep praying.
- Go back and finish the page. When the answer comes, write it in the praise report. If it looked different than you expected, write that down too.
Over time, that stack of finished pages becomes proof of what the Lord has done. On a hard day, flipping through it is a real encouragement, especially when the thing you're praying about is taking a while.
And if you're praying for someone else, that works too. Put their name in the request box and write the praise report when you hear the good news.

What to print it on
You don't need anything fancy, but a few basics make the page nicer to write on. These are the Amazon links I'd start with (see the disclosure below):
- Heavyweight printer paper: thicker paper lasts longer, but plain paper will do just fine too.
- Fine-tip gel pens: smooth for writing out your verse and your requests.
- A 3-ring binder with sheet protectors: slide your pages in and you've got a running prayer list printable you can add to anytime.
